The commanderies of the Order are the subdivisions of the Johanniter Order based on regional affiliation.  They are either associations formed under the laws of a now defunct state, or registered associations.  Each commandery is headed by a Ruling Commander, who also represents it.  The Commander is appointed by the Herrenmeister. The Commander is assisted by the Convent.  Together with the convent, he supervises the institutions and facilities managed and operated by the commandery.

Wherever a sufficient number of members of the Order live in an easily surveyable geographic area, they will be brought together by the Commander of the regionally responsible commandery to form a sub-commandery, regardless of the commandery to which they actually belong.  The sub-commanderies serve the Order and its charitable works, ensuring the sense of belonging and spirit of community among the members and involving them in the work done by the Order.
