Our aim is that you are also well cared for at home! We see ourselves as a link between our hospital stay and subsequent care. Through this networking, every patient who needs nursing care or treatment after discharge should receive it without delay. During discharge management, you and your relatives are comprehensively informed and discharge is carefully coordinated and organised. Right at the beginning of your stay, we contact you and your family and inform you about your current situation. During the course of your stay, we will then clarify - during consultation with the attending doctors and nursing staff - what need there is for further care. Together we discuss these possibilities and then take the appropriate measures.

In geriatrics, “case management” is responsible for this organisation. It accompanies the patient through their stay and is informed about how the patient’s case develops throughout therapy, via regular team meetings. Here, too, individual care services are worked out and implemented, with a focus on maintaining the greatest possible independence.
