Patient Discharge
Within the framework of discharge management at Johanniter-Krankenhaus Geesthacht, we ensure needs-based and continuous care following your inpatient stay. Our team of doctors, nurses, and social services will provide you and your relatives with comprehensive information and advice.
The most important points on discharge
- Already, during the admission interview on the ward, doctor and nursing staff assess your therapeutic and nursing care needs for the recovery period following your discharge and, at an early stage, inform the doctor who will be providing you further treatment.
- During your stay, social services provide assistance in applying for aid, for a certain level of care, or in organising an appointment for follow-up rehabilitation.
- On the day of your discharge, you will receive a discharge letter informing you and the doctor or institution providing further care, in detail, about your course of treatment and the recommended follow-up care.
- Johanniter-Krankenhaus Geesthacht has two professional nursing trainers who prepare relatives for any new home care situation if required and desired. Care training can begin at the hospital bedside and be continued within the home environment immediately after discharge.
Discharge management helps with planning
JohanniterKrankenhaus Geesthacht carries out discharge management in accordance with § 39 Para. 1a SGB V.
Why is a statement of consent required?
As part of discharge management, the hospital may need to contact, for example, doctors, healthcare providers, such as physiotherapists, or suppliers of medical aid and/or the patient's health or long-term care insurance fund. For this purpose, it may be necessary to have to transmit the patient’s data, which requires the written consent of the patient.
This can be done by means of this consent form, with which patients can declare their consent to discharge management and the associated data transfer, as well as to the support of discharge management by the health/care insurance fund and this associated data transfer as well.
